Registration for Sports Made Easy

Huntington UFSD is now partnering with FinalForms, an innovative online forms and data management service that promises to revolutionize the current athletic program form submission process during each sport season at J. Taylor Finley Middle School and Huntington High School.

With FinalForms, parents can conveniently complete and sign athletic participation forms for their child using any digital device. This user-friendly platform also empowers parents to review and update their personal information at their convenience, ensuring its accuracy.

FinalForms even pre-fills information wherever possible for each student, effectively saving everyone valuable time. One of the most valuable features of FinalForms is its ability to retain data season after season and year after year, eliminating the need to enter the same information repeatedly.

Parents will be able to upload physical exam reports, interval health history documents and digitally sign consent forms. Student-athletes will be asked to sign up for their respective sport and sign-off on code of conduct and consent forms.

As the Huntington athletic department and nurses work to seamlessly integrate FinalForms into its systems, the district a Parent Playbook (link below) that offers clear instructions on how to register and access an account and begin using FinalForms. Should parents have any questions or require assistance regarding this partnership they can reach-out to athletic office assistant Nicole Haas at 631.673.2017 or nhaas@hufsd.edu.

The Parent Playbook will get you started in FinalForms. If you require any support during the process, scroll to the bottom of the web page and click “Use Support.”

A Longstanding Tradition of Excellence

Through the years, the Huntington interscholastic athletics program has played an important role in the lives of students and community members. An integral part of the Department of Physical Education, organized athletics in the Huntington School District date back more than one hundred years. Since the inception of sports teams here, Blue Devil competitors have won every possible honor bestowed on high school athletes.

Life Lessons and Opportunities to Participate

Students who choose to participate will learn lessons in the value of hard work, teamwork, personal sacrifice and commitment, time management, respect for authority and rules, as well as how to lead and follow. A myriad of sports are offered during fall, winter and spring seasons. Students can choose to compete on one team each season. Parents of athletes, alumni, and community members who want to support and become involved in the athletic program have an opportunity to join the Huntington Booster Club. This organization sponsors many activities and promotes and enhances the operation of the athletic program.
