Final Forms is Open for Fall Varsity-JV Athletic Signups

The Final Forms online athletic registration and sign up system is now completely open to any Huntington High School student who would like to play on a fall season varsity or junior varsity team.

Final Forms allows parents to upload physical exam paperwork and complete and sign interval health history, athletic participation and parent consent forms for their child. Final Forms saves data from season to season and year to year so parents will never need to enter the same information twice.

Parents should visit the online Huntington UFSD Final Forms site: https://huntington-ny.finalforms.com. If you played on a team at Finley or the high school last year, there is no need for re-registration.

For those new to the district and/or athletic program, parents can create their account today. The Parent Playbook available on the district website provides detailed directions on how to enroll as a parent/guardian as well as how to register student-athletes.

The Huntington varsity and junior varsity football teams begin practicing on Monday, August 17. All other Blue Devil fall sports teams will hold their first practice/tryout sessions on Monday, August 24.

Final Forms is the only way to sign up for a fall season sport. If parents require any support they should scroll to the bottom of the Final Forms page and click on “Get Help” or send a message to Blue Devil athletic office assistant Nicole Haas at nhaas@hufsd.edu. Check the Parent Playbook, too.

Please note if you are creating an account: During the registration process, Final Forms will request General Student Information, including a unique student email address, which must be different than the parent/guardian email address. Parents can either use their child’s personal email address or their child’s Huntington UFSD email address.
