Final Forms Opens for Spring Sign-ups on February 18

Final Forms opens for spring season sign-ups for athletes at Huntington High School on February 18. Athletes at J. Taylor Finley Middle School can begin signing up on February 25. This will mark the full implementation of Final Forms in Huntington UFSD.

Parents of athletes in grades 7-12 must register for a free account and sign their child up for sports through the account. It is the only way to sign-up for a spring season sport.

Final Forms is an online athletic registration system, which will streamline athlete sign-ups, boost rules and safety related compliance and reduce repetitive paperwork for families.

Final Forms allows parents to upload physical exam paperwork and complete and sign interval health history, athletic participation and parent consent forms for their child. Final Forms saves data from season to season and year to year so parents will never need to enter the same information twice. It will pre-populate information wherever possible for each student, thus save parents time.

Entering this week, 502 parents have registered 323 students athletes, including 179 at the high school and 144 at Finley Middle School. The only way to sign-up for a spring season team is through Final Forms. It is important the parents create an account now.

Parents should visit the online Huntington UFSD Final Forms site: https://huntington-ny.finalforms.com. Parents can create their account today. The Parent Playbook available on the district website provides detailed directions on how to enroll as a parent/guardian as well as how to register student-athletes.

Once parents establish their free account with Final Forms, the will receive reminder emails to sign up their child for a sport during each season.

The spring season begins at Huntington High School on Monday, March 17 and at Finley Middle School on Monday, March 24. During the registration process, Final Forms will request General Student Information, including a unique student email address, which must be different than the parent/guardian email address. Parents can either use their child’s personal email address or their child’s Huntington UFSD email address.
