Parents of Athletes Asked to Establish Final Forms Accounts

The parents of Huntington UFSD athletes in grades 7-12 are being asked to establish accounts on the Final Forms website platform now. The Blue Devils are in the midst of transitioning to the online registration system used by school district throughout Suffolk and Nassau and across New York State. It promises to make things easier on parents and athletes by streamlining the sports sign-up process, including allowing forms to be submitted and signed off on digitally.

Stylized as FinalForms, the new system allows parents to save data from season-to-season so there is no need to keep entering the same information. Parents can review and update data as needed. Consent forms can be signed digitally along with other required paperwork, including interval health history forms and physical exam reports. Any digital device can be used to access Final Forms.

The Blue Devils hope to go live with the new system for the winter season. That’s why parents need to register and create an account now. Once the account is created, there is never a need to create one again.
