Final Forms is Coming to Huntington UFSD

July 22, 2024

Huntington UFSD is partnering with Final Forms (stylized as FinalForms), an innovative online forms and data management service that promises to revolutionize the current athletic program form submission process during each sport season at J. Taylor Finley Middle School and Huntington High School.

With Final Forms, parents can conveniently complete and sign athletic participation forms for their child using any digital device. This user-friendly platform also empowers parents to review and update their personal information at their convenience, ensuring its accuracy.

Final Forms even pre-fills information wherever possible for each student, effectively saving everyone valuable time. One of the most valuable features of Final Forms is its ability to retain data season after season and year after year, eliminating the need to enter the same information repeatedly.

Parents will be able to upload physical exam reports, interval health history documents and digitally sign consent forms. Student-athletes will be asked to sign up for their respective sport and sign-off on code of conduct and consent forms.

As the Huntington athletic department and nurses work to seamlessly integrate Final Forms into its systems, the district will soon provide a Parent Playbook that offers clear instructions on how to register and access an account and begin using Final Forms. Should parents have any questions or require assistance regarding this partnership they can reach-out to athletic office assistant Nicole Haas at 631.673.2017 or nhaas@hufsd.edu.

Once the transition to Final Forms is complete later in the school year, all parents and student will be asked to embrace it as part of Huntington UFSD’s streamlined approach to paperwork.
