ImPACT Concussion Baseline Testing Available

Any Huntington High School student-athlete in a fall contact sport (football, soccer and field hockey) is invited to take the ImPACT concussion baseline assessment. This is a free service for student-athletes.
The assessment evaluates the brain’s ability to remember words and symbols as well as reaction time. While there is no grade given for the assessment, an athlete should complete each part of the evaluation as quickly and as correctly as possible. There is a 45-minute time limit.
Those who complete the evaluation will know it has been recorded successfully when they get to the page asking if they would like to print or email their results. There is no need to do so, they may simply close out the program once they reach that page.

• Enter customer code: CPDX84SMTK (does not need to be all caps)
• Click VALIDATE under Huntington UFSD NYSIR click LAUNCH TEST
1. Select preferred language, click NEXT
2. Read instructions and ensure your environment is comfortable. A mouse or the track pad must be used (Do not use touch screen), click NEXT
3. Enter your first and last names as they appear on your report card. Enter birthday, gender and indicate whether you are using the track pad or a mouse. Click NEXT
4. Answer the (3) questions. If you have been diagnosed by a medical professional with a concussion in the last six months, please indicate so where asked. Click NEXT
5. Click ENTER ADDITIONAL DEMOGRAPHICS
a- NATIVE COUNTRY, language and secondary language if desired then click NEXT
b- YEARS OF EDUCATION COMPLETED: senior-11, junior-10, sophmore-9, freshman-8, 8th grader-7 AND answer the questions following to the best of your ability then click NEXT
c- CURRENT SPORT only, then click NEXT
d- please enter the # of times diagnosed with concussion, (if you click 1 or more, there are additional questions to be answered) then click NEXT
e- answer the (7) questions, then click NEXT
f- answer the (3) questions, then click NEXT
g- enter the approximate date of your last diagnosed concussion, if applicable, then click NEXT
6. The next (4) pages will ask about your current symptoms and conditions. You must choose a number indicating how you are currently feeling for each symptom before you are allowed to move on. Once NEXT illuminates you may click it.
You will now begin the evaluation. There are instructions and samples before each portion. Please reach the instructions and complete all sections within 45 minutes.
You will know that you have finished when you reach the page asking if you would like to print or email your results.
