Huntington UFSD’s athletic program is preparing to restart next month as official practices commence for winter season sports at both Huntington High School and J. Taylor Finley Middle School.
A series of important dates have been released along with a lineup of sports for each season.
Winter season athletes must submit a health history form and notify the school nurse of the sport they will be trying out for prior to December 7 at Huntington High School and December 14 at J. Taylor Finley Middle School.
Winter season athletes at Huntington High School who have not had a physical exam by their private physician on or after January 4, 2020 can get a physical exam from the school doctor on Monday, December 14 at 2:45 p.m. All students should report to the gymnasium and they will be brought to the nurse’s office for the physical exam. (Students on remote learning that day must go through the temperature check in the gym lobby that day at 2:45 p.m.)
Winter season athletes at J. Taylor Finley Middle School who have not had a physical exam by their private physician on or after January 4, 2020 can get a physical exam from the school doctor on Monday, December 21 at 3:15 p.m. All students should report to the gymnasium and they will be brought to the nurse’s office for the physical exam. (Students on remote learning that day must go through the temperature check at the rear entrance that day at 3:15 p.m.)
- In-person students must bring a copy of their physical exam form and completed health history form to the school nurse during the day at the high school or during their lunch period at Finley.
- Virtual/remote students have three options to submit their forms: Put forms in an envelope address to the school nurse and drop it off at the main entrance; FAX forms to the Finley nurse at 631-425-4746 or the high school nurse at 631-425-4725; e-mail the forms to Finley nurse Kay Kokhar (kkokhar@hufsd.edu) or high school nurse Linda Amarante (lamarante@hufsd.edu). Include student-athlete’s names and their winter sport on all correspondence.
No one will be allowed to participate without presenting an up-to-date physical exam report, signed parent/guardian permission/code of conduct slip and a current health screening form. These documents are available in the athletic office at Huntington High School during daytime hours or via downloading on the school district’s website at www.hufsd.edu – there is a link to the documents on the homepage under Informative Links or click on the Main Navigation on the top left of the homepage and then on Fall Season under Athletics. Scroll down to Documents and click. A dropdown menu of items will appear.
